a free reflective worksheet

What do you want for yourself?

A ten-page reflective worksheet from Jay Mullins ChangeWorks. A quiet place to think honestly about the life you're living, and the life you may still want.

what's inside

Two parts. Ten pages. One honest look at your life.

  • Part one, the life you're living: guided questions and sentence completions on what feels aligned, and what continues out of habit.

  • Part two, the life you may still want: space to consider what still feels possible, beyond the life you already know.

  • A closing reflection on what could come next, if you let it.

A full life is not the same as a fully lived one.

Meet Jay

I created The ChangeWorks Program after spending more than two decades leading change professionally and navigating significant change personally.

Over the years, I came to understand something deeply.

Change itself is rarely the hardest part.

The real challenge sits underneath it: the habits we build, the beliefs we hold, the patterns shaping our decisions and the internal resistance that keeps us attached to what feels familiar.

As I began asking bigger questions about what I wanted for my own life, I realised women need more than reflection when they reach this point.

We need structure. A way to think clearly about where we are now, what matters most and what it will take to intentionally create meaningful change. What I couldn't find was a process to do that.

So I built one.
